WebWraparound REACH Care Coordination Program. This program is voluntary for Wellpoint Care Network families who have a young person with serious mental or emotional health needs that affect the youth’s ability to function at home, school or in the community, but who is not yet involved in the child welfare or juvenile justice systems. WebWraparound is a planning process that follows a series of steps to support youth and their families accomplish the goals needed in order to reach their vision. Wraparound is a trauma informed practice that begins with getting to know the youth and family to learn about their strengths, needs, and culture.
